The Institutional Review Board (IRB) at the NYU School of Medicine promotes and protects the welfare and rights of all human research participants. The NYU SoM IRB facilitates excellence in human research by extending personalized guidance and support to the research community through timely and high quality service, education, review and monitoring for all human projects conducted at NYU SoM.

 

Dear Research Community,

In an continuous effort to streamline the informed consent process and provide subjects with comprehensive informed consent documents (ICD), the NYU LMC/SoM Institutional Review Board office along with senior leadership evaluated the current ICD template and revised the document content.

New Informed Consent templates are now available on the IRB web site. See link at http://irb.med.nyu.edu/researchers/library/templates

There are two templates; one for Biomedical Consent Form Template and one for Non Biomedical Consent Form Template. The templates include standard institutionally required language and each template is accompanied by a companion document. The companion document provides sample language suggested by the NYU SoM Institutional Review Board. Though the suggested language is not required, if replaced by other language please make sure consent forms are written at an 8th grade reading level to ensure comprehension.
